Waking up with a deep, throbbing ache in your backside that shoots down your leg is an alarming experience. For most people, the immediate assumption is that they have developed a severe case of sciatica. They head straight to Google, read about slipped discs, and panic that they might need lower back surgery.

Understanding True Sciatica
True sciatica is a very specific medical condition. It occurs when the sciatic nerve is compressed or irritated at its very root, right where it exits your lower spine.
Your sciatic nerve is the longest and thickest nerve in your human body. It is formed by a cluster of nerve roots exiting your lower lumbar spine and upper tailbone. In cases of true sciatica, a structural issue in the lower back is usually to blame. This could be a bulging disc pressing backwards onto the nerve root, or a narrowing of the spinal canal known as stenosis.
Because the nerve is being pinched at the source, the symptoms of true sciatica are usually highly neurological and follow a very specific path.
Understanding Tight Glutes and Piriformis Syndrome
On the other hand, you have your gluteal muscles. Deep underneath your main glute muscles lies a small, diagonally shaped muscle called the piriformis.
In most people, the thick sciatic nerve runs directly underneath this piriformis muscle. If you spend long hours sitting at a desk in Wellington, or if you have rapidly increased your running or lifting volume, this piriformis muscle can become incredibly tight and inflamed. When it spasms, it clamps down directly onto the sciatic nerve.

It is described as a sharp, burning, or sudden shooting pain, often accompanied by a loss of physical strength in the foot. Gluteal referred pain usually feels like a deep, heavy, and exhausting muscular ache that sits right in the centre of your buttock cheek.
Finally, pay attention to your mechanical triggers. True sciatica is often made sharply worse by bending forward at the waist or coughing, as these actions increase pressure on your spinal discs. Piriformis pain is usually triggered by sitting on hard surfaces for extended periods, or by climbing stairs, as these actions directly stretch and compress the gluteal muscles.
