What is Erectile Dysfunction

Erectile Dysfunction (‘ED’) is the persistent and/or re-occuring inability to achieve and maintain an erection firm enough for a consistently healthy sex life. Its a profoundly life impacting condition that can cause a great deal of stress

Erectile dysfunction is estimated to impact over 50% of men aged 40-70, and a great many more are affected by transient erectile dysfunction, on a more occasional basis.

While most men occasionally struggle to get or maintain an erection this isn’t necessarily a cause for concern. Our bodies don’t always perform at 100%, especially when we are exposed to things like alcohol, stress, unhealthy foods and even poor sleep.

Erectile can cause stress, affect self-confidence and contribute to relationship problems. So it’s important not to trivialise or normalise the condition. Erectile Dysfunction is no joke.  In some rare cases, it can even be a sign of serious underlying health conditions. This is why it is always worth getting some medical checks done before you undertake treatment for erectile dysfunction. Your doctor won’t be able to offer much in the way of treatment but they can rule out the more serious possibilities that free you up to work on your ED with a clinic that specialises.

If you are looking to get in better shape the best way to approach this is gradually and incrementally. Our society is notoriously bad for blaming very treatable issues as being just age-related. At almost any age we can make improvements to our health and reap the benefits.

Penile injection therapy involves the unappealing act of injecting medication directly into the side of the penis, typically using a fine needle. These medications, such as alprostadil (Caverject, Edex), help relax blood vessels and increase blood flow to the penis, resulting in an erection that can last up to 1 hour. Penile injections are an effective option for men who do not respond to oral medications or cannot tolerate their side effects.

Naturally many of us men don’t feel too excited about the prospect of jabbing ourselves in the penis every time we want to have sex. Yet, if that is ultimately the only option so be it. The love life is important.

Vacuum erection devices (VEDs) are non-invasive devices that create a vacuum around the penis, drawing blood into the shaft and producing an erection. For assisting with erectile dysfunction. Once the erection is achieved, a constriction ring is placed at the base of the penis to maintain it. VEDs are safe and effective for many men with ED and can be used as a standalone treatment or in combination with other therapies. This is another area of erectile dysfunction management that is far less than ideal and threatens to be a ‘mood killer’all unto itself. But like penis injections sometimes we just have to do whatever it takes to maintain our sex life. And nothing that works should be ruled out, especially if it is safe.

For men with severe or refractory ED, penile implants offer a long-term solution. These devices are surgically implanted into the penis and allow for on-demand erections by manually activating the implant. Penile implants come in two main types: inflatable implants, which mimic the natural erection process, and semi-rigid implants, which maintain constant firmness. While surgery is required to implant these devices, they provide a high level of satisfaction for both patients and their partners. This isn’t as ideal as restoring healthy erectile function to the penis, but anything that works should not be ruled out. As with most of what ails us, an exhaustive search for safe natural solutions should be undertaken before something as serious as surgery is considered.

Through a selective database search following PRISMA guidelines, 11 prospective randomized clinical trials (RCTs) from January 2010 to December 2020 were included, covering a total of 920 male patients with ED symptoms. While 348 patients showed a statistically significant improvement in erectile function after LI-ESWT, 572 did not. The review concludes that while LI-ESWT shows promise in laboratory studies.

Low-intensity Shock Wave Therapy for Vasculogenic Erectile Dysfunction: Technical Considerations and Treatment Outcomes

Summary: This narrative review examines low-intensity shock wave therapy (LiSWT) as a potential treatment for vasculogenic erectile dysfunction (ED). LiSWT, a form of regenerative therapy, aims to stimulate blood vessel and nerve regeneration in the erectile tissue. While basic science supports its mechanism, clinical evidence remains varied. Multiple randomized controlled trials and meta-analyses have assessed LiSWT's efficacy, showing promising results.
